Easy Steps to Navigate Connect®

1. GETTING STARTED

Go to www.mynuquest.com.
Click on Connect in the upper right corner.

If you are returning to Connect®:

  • Login to Connect® using your Username and Password.

If you are a new user:

  • Call a Service Coordinator for assistance at 866.858.7161 (option 2) or register online.
  • Look for the New Users box.
  • Click on Register Here.
  • Create a Username and Password by completing the registration form.
  • Click the Continue button to complete registration.
  • Select New if you are the first person to log in from your company then
  • Click Continue to complete your registration or if you are not the first person to register from your company then
  • Select Existing if the company's address is already on file and is shown on your screen.
  • Print your final registration information.
  • Submit to complete your registration.
  • Sign in using your new Username and Password.

2. WELCOME PAGE

From the Connect Welcome page, you will have access to tools enabling you to:

  • Submit a new referral: Top Tab New Referral. Click here for next steps.
  • Review real-time status of existing referrals: Top Tab Referral Inquiry. Click here for next steps.
  • Check detailed referral information: Top Tab Referral Inquiry. Click here for next steps.
  • Access required completed forms and releases: Top Tab Referral Inquiry. Click here for next steps.
  • Read helpful information to ensure compliance with MSP guidelines: Top Tab Referral Inquiry. Click here for next steps.
  • View or download published reports: Top Tab Referral Inquiry. Click here for next steps.
  • Upload documents: Top Tab Upload Documents. Click here for next steps.

3. SUBMIT A NEW REFERRAL

If you would like to submit a new referral:

  • Click on the New Referral tab to view the Referral Form.
  • Complete all required fields (indicated in red).
  • Request complimentary file COPY/PICK-UP service, if desired.
  • Submit the referral by clicking the Submit button on the bottom right of the referral form page.
  • Confirm receipt of the “Referral submitted successfully” notification.
  • Keep the notification handy as it contains the file number and will provide a list of documents or records needed to complete the service requested.

4. REVIEW REAL-TIME STATUS OF EXISTING REFERRALS

If you would like to review the status of your existing referrals:

  • Click on the Referral Inquiry tab.
  • Select New, Open and/or Closed options.
  • Enter the claimant's last name, first name or SSN, if searching for a SPECIFIC referral or
  • Search the database to view ALL referrals associated with your account.
  • Locate the file you would like to review.
  • Click on the Date Ref. link to access the tabs to the referral information.

Checklist

If you would like to review the detailed referral information:

  • Click on the Checklist tab.
  • Review the dates of completion to determine the current status of existing referrals.
  • Print a copy for your records by clicking Print Version at the top or bottom right corners of the page.

Referral Request

If you would like to check for the real-time status of the referral:

  • Click on the Referral Request tab.
  • Scroll to review the referral data in detail.

Forms

Completed Forms and Releases

If you would like to access completed forms and releases associated with the referral:

  • Click on the Forms tab.
  • Click on the desired form from the twelve choices in the top section of the page as shown below:

  • Print or Save a PDF copy for your records.

Blank Forms and Releases

If you would like to access blank forms and releases, stay within the Forms tab:

  • Scroll to the section entitled Blank forms are available below.
  • Click on the desired form from the twelve choices in the bottom section of the page as shown below:

Published Reports

If you would like to access the most recent MSA report and CMS submission report:

  • Click on the Published Reports tab.
  • Click on the desired form from the choices presented.
  • Print or Save a PDF copy for your records.

5. UPLOAD DOCUMENTS

If you would like to upload medical records, payouts, and other documents needed for the referral:

  • Click on the Upload Documents tab.
  • Click on the Browse button.
  • Attach the document(s) you want to send.
  • Scroll down to the Upload button.
  • Click on the Upload button.

Parameters for uploading documents:

Flash Upload:

  • Accepted file types include .doc, .xls, .jpg, and .pdf.
  • Individual file size should not exceed 50 MB.

Non-Flash Upload:

  • Accepted file types include .doc, .xls, .jpg, and .pdf.
  • Individual file size should not exceed 20 MB each and total transfer size not exceeding 80 MB.
